Please read this waiver agreement carefully before signing. By signing this document, you acknowledge that you understand and agree to the terms and conditions outlined below.
Determining the Nature of the Healing & Coaching Sessions: I understand that healing and coaching sessions are a collaborative process between Empress Ngala and myself (the client) designed to support, empower, and guide me (the client) in achieving my spiritual, personal and other specific goals. It is not a substitute for medical, psychological, or financial advice.
Client Responsibility: I acknowledge that I am responsible for my own decisions, actions, and results. Empress Ngala’s role is to facilitate the process, providing guidance and support, however I understand that my progress is determined by me.
Confidentiality: Empress Ngala will maintain the confidentiality of all information shared during coaching sessions, except where disclosure is required by law.
Release of Liability: I hereby release Empress Ngala AKA Roxanne Henry from any and all liability for any loss, damage, or injury that may arise from my participation in healing or coaching sessions. I understand that healing and coaching sessions are not guaranteed specific outcomes, and I assume all risks associated with the healing and coaching process.
Cancellation Policy: I understand and agree to the cancellation policy as outlined by Empress Ngala AKA Roxanne and listed in the Policy and Procedures section on the website innersoulwellness.center.
Agreement to Terms: By signing below, I acknowledge that I have read, understood, and agree to the terms and conditions of this waiver agreement. If you have any questions or concerns, please discuss them with Empress Ngala before signing this agreement. Thank you .

I understand that healing sessions or coaching are not therapy or counseling and do not substitute for professional mental health care or medical advice. I confirm that I am competent and capable of taking responsibility for my own decisions and actions. I release Empress Ngala AKA Roxanne Henry from any liability for any outcomes or consequences resulting from my participation in coaching.

[ ] I acknowledge that I am responsible for my own decisions and actions taken as a result of life healing and coaching sessions.
